About us – Aged Care Services Monash Health has four Residential Aged Care Facilities caring for 194 Residents, currently 104 Beds are dedicated psychogeriatric nursing home beds with 75 Beds in secure units.
The resident cohort Monash Health supports, live with significant complex and chronic physical, and mental health illnesses and activities of daily living care needs.
The services offer a holistic, person-centred care provided by a Registered Workforce mainly Registered Nurses (RN) and Enrolled Nurses (EN) with some sites supported by Psychiatric Services Officers (PSOs).
The Nurse Practitioner role has emerged in recent years as an important means of achieving significant outcomes for residents, patients, their families, and the health system.
These outcomes include increased client satisfaction with service provision, an increase in resident/patient access to services, and a decrease in the hospital length of stay and unplanned readmissions.
About The Role Operationally reporting to the Director Clinical Operations – Residential Aged Care, and professionally to the Director of Nursing - Aged Care Services.
Additionally, the Nurse Practitioner will work under the supervision of a Geriatrician and the primary responsibility is to provide support to Monash Health Aged Care residents and bed-based Transition Care Patients (TCP) referred by the Residential in Reach team.
The role is responsible for urgent review of residents with significant clinical deterioration that may have unstable observations including heart failure and intervention with severe behaviours that are new or unexpected when the treating GP may not be available.
You will also undertake non-urgent assessments of residents requiring geriatric syndrome review which may include severe behaviours of concern, falls risk management, complex wound management and medication management for new admissions.
Working within the wider multidisciplinary team you will provide in person clinical assessments, review and evaluations which will formulate modified care plans for the resident.
You will provide phone support for the wider Nursing team and develop strong relationships across all Monash Health Aged Care services.
The role is an ongoing 1.0 EFT Full Time position on a Monday – Friday roster within the working hours of 8.30am – 5.00pm.
However, these hours may vary depending on business needs.
