ABOUT THE ROLE We are looking for motivated Registered Nurses with highly developed clinical practice, decision-making, client advocacy and sound assessment skills to join the District Nursing team in a part time capacity.
We are a fast-paced leading provider of health care in the community who service a wide variety of clients that includes general nursing, chronic and complex care and palliative care.
This position will provide a flexible and timely District Nursing service focussed on support for older frail or younger disabled people (or their carers) to be more independent at home and in the community.
This support aims to enhance quality of life and/or preventing admission to an acute facility or inappropriate admission to long term residential care.
A willingness to work collaboratively across all Gateway Health public and fee for service funded programs is required as resources permit As a member of the District Nursing team you will: Be responsible for providing quality nursing care for a diverse group of clients Use a problem-solving approach involving assessment, planning, implementation and evaluation in the provision of quality nursing care to the clients Provide health education to clients and their carers about their identified health needs by creating an environment which enables them to be responsible for their own health Liaise closely with community service providers to coordinate client care Act as client advocate to assist positive health outcomes This position is part of the Rural Health Team, a multidisciplinary team delivering Allied Health and Nursing services.
This position will deliver District Nursing services within the Home and Community Care Program for Younger People (HACC-PYP) and Commonwealth Home Support programs and within the Fee-For-Service stream.
Services are provided in clients home and in the clinic setting.
The Rural Health Team services are delivered in peoples' homes, communities or are clinic based.
We use a mixture of individual and group service delivery models and have a focus on wellness and reablement.
ABOUT YOU We're looking for a dedicated nurse who excels in person-centred care and thrives both independently and as part of a team.
The ideal candidate will have extensive experience in wound care, palliative care, and general team nursing.
You will be a highly motivated and experienced professional, ready to collaborate effectively in multidisciplinary teams to deliver exceptional clinical services to our clients.
You will also bring: Current Registration with the Nursing and Midwifery Board of Australia.
