Job Summary
Wimborne and Ferndown PCN are looking for a keen individual to join our PCN Administration team to support our services to provide the best care to our patients. Our PCN includes the following practices: The Quarter Jack Surgery, The Old Dispensary, Walford Mill Medical Centre, Pennys Hill Practice, and Orchid House Surgery.
This role will be split between supporting the PCN Management team and wider PCN team to deliver on elements of the PCN DES and other initiatives as directed by NHS England; and to support the Ageing Well Nurse by assisting frail patients within our community.
Main Duties of the Job
The role is a mixture of administration and care coordination, supporting clinical teams in delivering the key performance indicators of the Network DES and other key network service areas. During the Covid-19 Vaccination Programme, the Network Care Coordinator will also provide administrative support for the vaccination hub.
The role will include:
Managing clinics to ensure each clinician is fully utilized.
Checking workload tasks and dealing with these in a timely manner.
Supporting PCN Management in delivering required services.
Taking meeting minutes.
Supporting clinicians to ensure smooth running of their clinics and bookings.
Booking/cancelling appointments.
Running reports on SystmOne and the DiiS.
Analyzing data.
Contacting patients via phone.
Working with the team to complete projects on health inequalities or other projects.
Triaging tasks and prioritizing appointment booking for the Ageing Well Nurse.
Checking practice's out-of-hour call lists for severe frail patients needing follow-up.
Accompanying the Ageing Well Nurse on visits to take notes as required.
Maintaining the Dorset Care Plan list and ensuring patients on this register are reviewed annually.
Forming good working relationships with our care homes.
Completing any administrative tasks required by the PCN Management team or Ageing Well Nurse.
Travelling between our 4 GP practices.
Overview of Your Organisation
Wimborne & Ferndown PCN provides services to 42,000 patients across 4 Practices (Quarter Jack Surgery, Old Dispensary, Walford Mill Medical Centre, Pennys Hill Practice, Orchid House Surgery). We have adopted new roles within the PCN to support and broaden our primary care team, allowing us to provide better care for our patients.
The PCN is headed by our committed Clinical Directors and a PCN Manager.
In addition to the PCN, our Practices are also part of Castleman Healthcare, a non-profit company owned by 13 GP Practices in North Poole and East Dorset (3 PCNs). Castleman Healthcare offers additional services to our patients through separate contracting arrangements, provides business and practical support to the PCNs, and supports representing the PCNs voice within the wider healthcare system.
