The Opportunity Core to the Amplar Health's strategy is substantial growth in our hospital substitution, hospital avoidance and clinical home care delivery. As such we have created a new role to offer you the opportunity to work as an OT in the Community as well as contributing to our Hospital and Rehab in the home programs as a Care Coordinator. You will receive full training and support in each element of your role, essentially expanding your clinical skills and being exposed to different parts of our business.
As an OT you will work with patients to:
Complete assessments including home safety assessments. Recommend equipment prescription/home modifications. Manage other relevant referrals. As a Care Coordinator you will work with patients via:
Monitoring and coordinating patients' treatment plans. Educating patients about their condition. Connecting patients with their health care providers. Evaluating their progress. As an Allied Health Care Coordinator for our Home Health business you could be part-time or full-time with approx. 2 days a week in Community and the remainder being in Care Coordination which can be done from the comfort of your home office.
About You As an unrestricted AHPRA Occupational Therapist we are looking for you to bring your experience into a community setting – and naturally you will need to be comfortable travelling in your own vehicle to our clients located in metro Adelaide (km reimbursement is offered). As a Care Coordinator this role will tap into your passion for customer service – as well as your strong interpersonal skills and engaging phone manner. A key element to establishing, monitoring, and coordinating patients' treatment plans will be comfort in utilising a variety of patient electronic medical systems and working closely with other scheduling, nursing, and provider teams.
