About the Role The NDIS Support Coordinator is responsible for coordination of all clients requiring support coordination, serviced by Roshana Care Group in the Perth Region.
The NDIS Support Coordinator will create partnerships with relevant parties in the Perth Region to build a portfolio of clients and in line with this growth build a support staff to provide the support required for these packages.
The NDIS Support Coordinator will create the support plans and risk assessments of the client and ensure that staff are providing care in support of this plan.
The NDIS Support Coordinator will provide services to link NDIS participants with providers best suited to them.
1x Part Time and 1x Full Time available.
Duties and Responsibilities Valued Based Practice Acts consistently in accordance with the Roshana Group Values and Key Principles – challenges practice inconsistent with these values and uses values as a basis for managing relationships and decision making.
Partnerships and Agreements Links Roshana NDIS participants to a 'greater community' that actively promotes the inclusion of any professional group to work with Roshana NDIS Services toward the provision of 'best-practice' outcomes that serve Consumers, Carers and related users.
Builds strong partnerships with mainstream supports and other NDIS registered and non-NDIS Registered service providers NDIS Business Development Network within the Perth Community to grow the business.
Work with the broader Roshana team to market the NDIS business in the Great Southern.
Create service agreements with clients and recruit to support these agreements.
Participants, Families, and Services Work with participants and families: Promote prevention, restoration and connection to create life skills and enable achieving goals Provide a model of Recovery that is individualized, trauma informed, family inclusive, person centered and (potentially) individual funding.
Promote holistic services that focus on total wellbeing, including physical health.
Attend NDIS package review meetings and assist clients to prepare for this.
Communicate with the participant's support coordinator and plan manager regularly, to streamline the services.
Create and regularly update support plans and risk assessments.
Psychosocial recovery coaching.
Engage with NDIA staff and LAC's.
Occupational Safety and Health Ensure the work environment is safe and healthy and complies with all OSH requirements.
• Take a shared responsibility to ensure the safety and well-being of themselves and others within the workplace.
• Work in a safe manner while exercising due care and caution.
• Report any unsafe practices or equipment • Wear/use all protective equipment provided and as instructed.
• Address breaches of OS&H policies with the individuals concerned, whether they are participants, family, Carers or other staff members Required Skills, Attributes and Experience: Essential: Minimum Certificate III in a health-related qualification.
Ability to develop ethical work practices including an understanding of appropriate personal boundaries.
Ability to work within a multidisciplinary team environment, promoting wellbeing and holistic health services Demonstrated high level written and verbal communication skills Demonstrated understanding of the person-centered approach Desirable: Knowledge and background with navigating SIL funding Knowledge of Nightingale CRM An understanding of contemporary NDIS Support Provisions.
Demonstrated proficiency in the use of a basic IT applications.
