Quality Health Care (QHC) is a specialist accommodation, disability, and community care provider with over 25 years' experience working with people to achieve their goals. We have a team of over 350 staff and we offer supported independent living, specialised disability accommodation, home care, support coordination, and Behaviour Support. QHC is looking for a Support Coordinator who can work with individuals, their families, friends, and supports to achieve set goals to improve the participants quality of life. Are you passionate about improving the quality of life for people living with disabilities? The successful candidate will be responsible for carrying their own case load, managing participants with varied disabilities and complexities. You will be delivering services to NDIS participants with Quality health care's approach to Support coordination in line with NDIS requirements. Service delivery is made up of a flexible mix of working from home, office based (in Hurstville) and client facing. We can offer A positive, professional, and supportive work environmentCareer progressionOngoing support, training, and developmentA dynamic team with heartRemote Position and work life balanceYour Role Assist participants in understand and utilising their NDIS plan funding. Work with participants to identify services that meet their needs and support their goals.  Coordinate a range of supports using a strength-based approach across all areas of the participants life. Ensure the participants have choice and control in selecting providers and build their capacity to self-direct. Manage ongoing support, including resolving issues between participants and providers.Liase with government and non-government bodies, oversee budgets, obtain quotes and submit progress reports to the NDIA.  Essential Criteria Relevant tertiary qualifications or certificate in relevant field e.g. Social work, community services or allied health.Experience in providing services to NDIS participants as a Support Coordinator or Case management experienceExperience in reading, understanding, and implementing NDIS plansExperience in connecting clients to services under a client direct care modelExcellent interpersonal and communication skills and a strong commitment to customer serviceAbility to manage a diverse participant portfolioAbility to maintain positive relationships with providers & community agenciesStrong customer service skills with demonstrated ability to develop and maintain relationships with people with disability and/ or their families and support network and service providersResilience and competence in high level interpersonal communication, negotiation and conflict resolution skillsSound time management skills and demonstrated ability to work with minimal supervisionDemonstrated ability to make complex judgements, prioritise tasks and take initiativeStrong written communication skills with the ability to write high quality reportsCurrent driver's license and willingness to travel to clients where requiredPrevious experience in a billable environment highly desirable Apply Now!!
