As we expand our services to include NDIS Support Coordination, we are excited to offer this new opportunity to join our growing team.
About the Role We are seeking a passionate and organised NDIS Support Coordinator in a part-time capacity to join our team.
In this newly established role, you will assist NDIS participants in navigating their plans and accessing the supports and services they need to achieve their goals.
You will act as a central point of coordination, empowering participants to exercise choice and control while ensuring the effective delivery of their supports.
Key Responsibilities Work collaboratively with participants to understand their NDIS plans and goals.
Assist participants in connecting with suitable providers, services, and community supports.
Monitor and review participants' services to ensure they are meeting identified needs and goals.
Provide ongoing support and guidance to help participants build capacity and independence.
Maintain accurate records and ensure compliance with NDIS standards and guidelines.
Act as a key point of contact for participants, families, and service providers to facilitate communication and resolve any issues.
Support participants in navigating service agreements and managing budgets within their NDIS plans.
What We're Looking For Previous experience in a support coordination, case management, or similar role (NDIS experience highly regarded).
Strong understanding of the NDIS framework, including the principles of choice and control.
Excellent communication and interpersonal skills to work effectively with participants, families, and service providers.
Strong organisational skills with the ability to manage multiple priorities.
A commitment to person-centred practice and empowering individuals to achieve their goals.
Proficiency in using technology and maintaining accurate documentation.
Relevant qualifications in disability, social work, or a related field (preferred).
