NDIS SUPPORT COORDINATORFIXED TERM PART-TIMEASPLEYYour Next Role!RFQ are currently recruiting a Part-Time Support Coordinator to join our NDIS team based in Aspley.As a Support Coordinator, you will support clients to achieve their goals in their Individual NDIS Plans and help bring each participant's NDIS plan to life.You will provide assistance to strengthen participants' ability to coordinate and implement their supports, assist people to strengthen their informal networks, and coordinate a range of services, including funded and mainstream supports.The position involves an understanding of the NDIS, the role of support coordination, and assisting supervisors with their supervisory responsibility, consistent with RFQ's recovery practice models.No day is the same, but it may include:Providing client support in accordance with RFQ's Model of practice including skills development, capacity building, and community connections, assigned tasks and activities as outlined in clients' schedules of support.Undertaking client assessments and reviews including risk assessments in accordance with RFQ's Model of Practice.Supporting clients to identify goals and support needs in line with NDIS Support Plans or other funding agreements.Maintaining oversight of individual support Service Agreements to ensure contractual obligations are met.You will need:Tertiary qualifications in social work, psychology, behavioural sciences, and/or a human services related field OR lesser qualifications with demonstrable experience.A strong sense of professional boundaries and integrity.NDIS Workers Screening, or ability to acquire.Knowledge and experience in the NDIS.Excellent organisational and time management skills.Demonstrated skills using client information management systems.The ability to build rapport quickly and effectively with multiple stakeholders.Current driver's licence and access to a registered roadworthy and insured vehicle (minimum third-party property).Why RFQ?At RFQ, we are committed to providing a supportive, collaborative, and empowering environment, where everyone can thrive.
