Aged & Disability Support (Community Services & Development) Nurture with Compassion is hiring a full-time NDIS Support Coordinator to provide services in the ACT and NSW regions.
The role focuses on coordinating and delivering tailored support plans to enhance the lives of individuals with disabilities or aged care needs.
Key Responsibilities: Developing person-centred plansOrganizing support servicesBuilding community networksCollaborating with clients and care teamsMonitoring client progressMaintaining NDIS-compliant recordsAdvocating for client rightsContributing to service improvementsRequirements: Applicants should hold relevant qualifications, have at least two years of related experience, possess strong communication and problem-solving skills, and understand the NDIS and aged care systems.
