About the Role Operating within the Mission, Vision and Values of Cocoon SDA Care, the National Care Coordinator is responsible for coordinating care and providing clinical guidance to teams comprising Service Managers, Team Leaders, Registered and Enrolled Nurses, and Disability Support Workers, to ensure the provision of high-quality best practice clinical care to residents and participants of Cocoon SDA Care.
The National Care Coordinator also provides training and supervision of a delegated worker to respond to the complex care needs of residents.
This role will provide support to the National Quality and Compliance Manager in ensuring CSDA's compliance with legislative and statutory requirements and NDIS high intensity skill descriptors, to enable a safe and high-quality standard of care and services to our residents.
This role will work with a variety of residents providing physical or psychological advice when required and support staff in the management of any clinical issues, complications, concerns, escalations, or emergencies in relation to a participant when delivering support.
Be a contributing member of the Care/Support Team and Compliance Team.Provide clinical advice to the Care/Support Team and Compliance Team.Assist in identifying service gaps and improvements and lead ongoing continuous improvement to achieve clinical best practice.Communicate effectively and build strong relationships with Residents and Participants and other Support/Care Team Members.Assist in developing a capable Care/Support Team through coaching, mentoring, training, supervision, and conducting competency assessments to meet resident needs effectively and safely.Promote a culture of service delivery that supports the Participant or Resident's choice and independence.Support the implementation of Cocoon SDA Care's clinical governance framework.Support Residents and Participants with complex needs and behavioural concerns through responsive and flexible case management aligned with resident and participant goals and continual support towards recovery and positive outcomes for participants.Oversight of High Intensity Support Plans and assessments.Assist in conducting Resident or Participant re-assessments, reviews, and providing ongoing support to the participant, workers, and other stakeholders.Coordination of clinical incidents, risks, and complaints raised by staff, external stakeholders, and the NDIS Quality and Safeguards Commission.Visit Resident or Participant homes and check clinical compliance and quality.Support the Resident or Participant to prepare for their next NDIS Review Meeting, i.e., preparation of nursing assessment/report.Maintain professional knowledge and continue to develop knowledge and expertise in relation to professional skills and professional regulatory requirements.Review and develop clinical policies, procedures, processes, and workflows and participate in NDIS internal and external audits.Other Duties: It is reasonable to expect that this job description serves to illustrate the scope and responsibilities of the post and is not intended to be an exhaustive list of duties.

Minimum Qualifications and Requirements: Current unrestricted Registered Nurse Registration as approved by the Australian Nurses and Midwifery Board, i.e., Membership with Australian Health Practitioners Regulatory Authority (AHPRA).Current NDIS Worker Screening Check.NDIS Mandatory Worker Orientation Certificate of Completion.Working with Children Check (WWCC) clearance within the last five (5) years.COVID-19 Infection Control Certificate.First Aid, CPR, and Anaphylaxis Training.100 points of identification.Right to work within Australia.Current Driver License and associated Insurance.Why Work for Us?
