Support Coordinator/MND Advisor - Fraser Coast Region $49.28 per hour + super + salary packaging
We are seeking a highly organised, experienced, empathetic, and positive Support Coordinator who is interested in specialising in Motor Neurone Disease (MND). You will have the opportunity to work closely alongside other MND Advisors/Support Coordinators and be well supported by our Team Lead. You will be supporting clients with MND located in the Fraser Coast region, including Bundaberg, Hervey Bay and beyond and can therefore be based anywhere in this region.

About the Role This is a permanent part-time position (0.6FTE - 22.8hrs/wk). It can be based anywhere in the Fraser Coast Region.
This role will be both rewarding and challenging. We support people at an incredibly difficult time in their lives, and in doing so, we are able to make a real difference at a time when support is most needed. You will ensure that our clients receive the vital services that are available to them, and reduce the reliance on informal supports and therefore the impact on their family members.
You are required to have a high level of autonomy as you will be responsible for managing your own caseload with support and guidance from our Team Leader. Whilst we work exclusively with MND clients, we take a highly individualised approach in our work to ensure clients receive the care they want/need, when they want/need it.
The role will see you:
Meet and engage with clients to provide information and advice following an MND diagnosis Assist clients to understand and access funding Provide pre-planning support to clients that are entitled to NDIS funding Assist NDIS funded clients to understand their NDIS plan Assist clients to utilise their allocated funding to meet their goals Effectively coordinate a client's care team to ensure all supports delivered to the client are focused on their goals, including working with public health, support workers, allied health, and medical practitioners. Work with clients to build capacity and independence Maintain up to date and accurate case notes relating to client supports Maintain a thorough understanding of NDIS funding requirements, systems and practices Maintain an understanding of other funding sources to support clients not able to access NDIS funding such as My Aged Care Develop knowledge of local service providers and Allied Health Professionals This is largely a Work from Home position. There is an expectation that you regularly provide face-to-face supports to your clients who will typically be based in Bundaberg, Hervey Bay, and surrounding areas. You will also be expected to come to the Oxley Office periodically for team days and professional development.
About You You will be highly organised, self-motivating, and proactive in finding solutions! You will have exceptional attention to detail and can be counted on to be reliable and true to your word.
You will be an experienced Support Coordinator with a strong understanding of the NDIS.
You will have strong communication and negotiation skills. Most importantly, you will be a people person; friendly, caring, and supportive.
Ideally, you will have experience in working with progressive neurological conditions and/or palliative care, and/or be proactive in upskilling and developing your knowledge and skills in these areas.
