Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual's needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job DescriptionThe opportunity:
Working at least three days, our in home support workers provide a range of care and services designed to meet our clients' needs and goals in a flexible and supportive manner. The role may include assistance with personal care, home care tasks, meal preparation, transport, socialisation and other responsibilities. Support work will be carried out across our three local regions in Whitehorse, Nillumbik and Manningham municipalities.
Desired Skills and ExperienceAre passionate, caring and committed to supporting people in their own home to maximise their quality of lifeAre able to assist in all activities of daily living- based upon the clients needs and goals- including personal care, domestic assistance, escorts to appointments, socialisation activities, meal preparation and more!Are able to work independently in the communityAre committed to the rights and dignity of peopleHave excellent communication skillsHave a current driver's license & well-presented insured carCan complete documentation of tasks on a smartphoneAre eligible to work in Australia & can provide a satisfactory National Police CheckCPR and First Aid CertificatesHold or can obtain a satisfactory NDIS Worker Screening Clearance and Working with Children CheckHave a qualification in and a passion for Aged Care or Community CareComply with OH&S instructions, Policies & Procedure, ensure timely and accurate collection of appropriate data to meet relevant reporting requirements ensure compliance with privacy legislationAvailable to work a minimum of three daysPlease apply here by submitting a cover letter addressing the key selection criteria with your CV.
