We are committed to providing older people who are at risk or have experienced homelessness with a range of high-quality care, housing, and support services. We employ more than 1,000 staff across Victoria and Tasmania.
Your new role We are currently seeking a proactive individual to join our Housing and Homelessness Team as a Housing and Homelessness Support Coordinator located at the Maude Street Office in Shepparton on a Full time Basis. This fixed term role will run from the 16th of December 2024 to the 24th of January 2025.
This position will work collaboratively with the other HHS Coordinators and HHS Managers to deliver Wintringham's Care Finder and Housing Support and Outreach services.
Your responsibilities and duties will include: Demonstrate leadership across the HHS portfolio and work collaboratively with all areas of Wintringham to ensure coordinated service responsesEstablish and maintain positive relationships across community and health services across the region.Promote a non-institutional philosophy within the programs by recognising and promoting the rights of Wintringham clientsOversee and ensure compliance to relevant legislative standards and guidelines across the relevant programsReview client file documentation across program areas to ensure the timely and accurate recording of information and monitor service delivery for qualityAdvocate and support clients to access private rental properties through internet searches and house inspections.Provide assistance to clients needing resources to access the private rental market, including bond applications, rent in advance and assistance with rent.Monitor targets in line with funding service agreements and ensure staff are reporting hours and support periodsWhat we will require from you: You will have a relevant tertiary qualification in Social Work, Welfare or Community Services or equivalent, current drivers license, and first aid (level 2), as well as:
Demonstrated experience in a team leadership role supporting clients experiencing homelessness with varying levels of complex psychosocial needs.Demonstrated experience working and leading within a case management/case coordination frameworkProven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing supportExperience delivering services within social and community services programs including housing and homelessness support services.Knowledge of Commonwealth and State government policies, standards and compliance requirementsWell-developed communications skills and the ability to engage people from varying backgroundsAn understanding of the Housing and Homelessness Service sector, My Aged Care, NDIS and other relevant service and regulatory systems.This sounds great!
