As a District Nurse with Empower, you will be responsible for the delivery of Empower Healthcare's comprehensive services across home care clients in the Southern and Eastern suburbs of Melbourne. You will:
Conduct comprehensive clinical assessments and develop clinically appropriate care plansProvide a range of nursing services to clients within the community, including but not limited to Medication Management, Wound Care, Diabetes Management, Catheter Care, Stomal Therapy and Cognitive AssessmentsWork holistically with other healthcare professionals to provide a multidisciplinary approach to client careEducate clients and their families to optimise the client's ability to live safely within the communityWork collaboratively with Case Managers and NDIS Support Coordinators to provide positive client outcomesWork autonomously to prioritise and complete tasks and complete all required documentation to a quality standardDo you have what it takes?We are looking for passionate people who care about making a difference in the lives of our people. You will have an enthusiastic and proactive attitude, be outcome focused and dedicated to delivering exceptional service to our clients. You must have:
Unrestricted Registration with AHPRA as a Registered NurseAvailability Mondays, Wednesdays and FridaysSound clinical knowledge and skillsExperience working with clients who have chronic and complex health needsExperience working with clients from diverse cultural and socio-economic backgroundsExcellent verbal and written communication skillsExcellent time-management skills and attention to detailNational Police Check ClearanceWorking with Children Check and NDIS worker screeningCurrent CPR CertificateWe ideally need our candidate to be available for 16 hours of coverage Mondays and Fridays each week, in South and East Melbourne.
