The Position
Our innovative service model focuses on delivering the best clinical outcomes for clients. As a District Nurse with Empower, you will be responsible for the delivery of Empower Healthcare's comprehensive services across home care clients in the Northern & Western Suburbs of Melbourne. You will:

Conduct comprehensive clinical assessments and develop clinically appropriate care plans
Provide a range of nursing services to clients within the community, including but not limited to Medication Management, Wound Care, Diabetes Management, Catheter Care, Stomal Therapy and Cognitive Assessments
Work holistically with other healthcare professionals to provide a multidisciplinary approach to client care
Educate clients and their families to optimise the client's ability to live safely within the community
Work collaboratively with Case Managers and NDIS Support Coordinators to provide positive client outcomes
Work autonomously to prioritise and complete tasks and complete all required documentation to a quality standard

You must have:

Unrestricted Registration with AHPRA as an Enrolled Nurse
Availability between Monday-Friday 8AM-4PM
Sound clinical knowledge and skills
Experience working with clients who have chronic and complex health needs
Experience working with clients from diverse cultural and socio-economic backgrounds
Excellent verbal and written communication skills
Excellent time-management skills and attention to detail
National Police Check Clearance
Working with Children Check and NDIS worker screening
Current CPR Certificate
Availability across Mondays, Wednesdays and Fridays highly desirable
