ABOUT THE ROLE Our Home Care service supports elderly residents to live independently in their own homes by providing a range of services tailored to their needs. These services include personal care, nursing, cleaning, and home care packages designed to empower clients to maintain their preferred lifestyle.
This role focuses on case facilitation and the coordination of client care services through a person-centered approach. The position involves working collaboratively with clients, their families, and other stakeholders to deliver comprehensive care planning, implementation, monitoring, and review.
KEY RESPONSIBILITIES: Developing and maintaining individual care plans with a clear focus on enablement and reablement goals.Facilitating effective interactions with clients, families, and service providers to ensure high-quality service delivery.Ensuring efficient and sustainable management of client care packages, including balanced budgeting and future planning.Advocating for and supporting clients in accessing the services they require.Promoting client health and well-being by conducting thorough assessments and tailoring care to meet individual needs.Maintaining compliance with all relevant policies, safety protocols, and consumer-directed care principles.The position will be offered to the successful applicant as a Max Term contract of 6 months with the potential to extend. January 2025 start, working 64hrs a fortnight - 4 days a week.
