Clinical Nurse Consultant or Clinical Nurse - Full time or Part time About the role
Holistic Nursing Solutions (HNS) is seeking a passionate and experienced Clinical Nurse Consultant or Clinical Nurse to join our team in Tamworth, New South Wales. This role is essential in supporting our community healthcare services and improving patient outcomes. We aim to provide flexibility, allowing the successful applicant to choose to work either part-time (0.6) or full-time, with full-time incumbents also responsible for coordinating our national continence services.
What you'll be doing Reporting to the NSW Clinical Manager, you will be responsible for:
Providing comprehensive nursing care and clinical expertise to patients within the community Assessing, planning, implementing, and evaluating personalised patient care plans Collaborating with multidisciplinary teams to coordinate holistic care for patients Monitoring patients' conditions and adjusting care plans accordingly Advocating for patients' rights and ensuring their needs are met Maintaining accurate and up-to-date patient records Participating in quality improvement initiatives and research projects Mentoring and supporting junior nursing staff What we're looking for Registered Nurse with a current AHPRA license Minimum 3 years' experience in community health nursing or relevant alternate experience Strong clinical assessment, decision-making, and problem-solving skills Experience in wound and continence assessment and management Excellent communication and interpersonal skills Willingness to promote our services in the community to secure new business Ability to work collaboratively in a multidisciplinary team Commitment to providing high-quality, patient-centred care Knowledge of relevant legislation, policies, and best practices Availability to work hours across at least 4 days of the week What we offer
