Our Health Independence Program helps clients manage their chronic disease in a proactive and planned model.
The Care Coordinator is responsible for identifying clients and working with them to coordinate the management of their chronic conditions. Working as part of a multidisciplinary team, along with external health care providers to coordinate chronic disease management.
