We are committed to providing older people who are at risk or have experienced homelessness with a range of high-quality care, housing and services. Your new role We are currently seeking a flexible and enthusiastic HHS Case Worker (HSO & CareFinder) to join our regional site based at our Hume Office in Shepparton on a Casual basis for a minimum of 7.6 hours weekly (up to 30 hours weekly). We are seeking candidates with excellent communication skills who can build rapport with those who are socially isolated within the community. You will have a non-judgmental attitude, the ability to liaise with people from varying socioeconomic backgrounds, and experience working with both socially isolated people and people with challenging behaviours. We offer flexible scheduling from 8:30 am to 5:00 pm to promote work-life balance and cater to individual preferences.

Your responsibilities and duties will include: Understanding and adherence to Wintringham's Code of Conduct. Ensure duty of care to both clients and staff is adhered to at all times. Ensure privacy and confidentiality is adhered to at all times. Complete/review client file documentation across program areas to ensure timely and accurate recording of information. Monitor service delivery for quality. Utilize relationships within sectors to assist access into other services and systems. Promote a non-institutional philosophy within the programs by recognising and promoting the rights of clients. What is in it for you? We are looking for candidates with the following: A qualification in Social Work, Social Sciences, Welfare or equivalent. Outreach skills including the ability to locate and establish rapport with both socially isolated people and people with challenging behaviours. Experience in working effectively and sensitively with a broad range of clients, including those with high support needs and challenging behaviours. An understanding of the Housing and Homelessness Service sector, My Aged Care, NDIS and other relevant service systems. A current driver's licence and reliable vehicle. Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
