We're looking for a dedicated Health Care Navigator to join our team in Wagga Wagga, NSW (Wiradjuri Country) on a part-time (30 hours per week) basis up until 30 June 2025!
In this role, you will collaborate with our care coordinators in addressing the diverse social needs that influence health outcomes.
You will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation.
