Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.
Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire, our 200 staff and 70 volunteers are based across four sites at Kyneton, Romsey and Sunbury.

About the Role Our Service and Care Coordination team is part of the Community Connections unit that sits within the Operations division. The Service and Care Coordination team has a vacant position for a Service Coordinator working with people who hold a Home Care Package.
This role will be responsible for providing support to the Senior Care Advisor, Care Advisors and Home Care package holders with a broad range of data service and administrative functions. The Service Coordinator will work in partnership with Senior Care Advisor and Care Advisors to make a positive difference to the wellbeing of Home Care Package holders in the community. You will be the first point of phone contact for Home Care Package holders, which includes answering incoming telephone calls, emails, responding to voicemail messages, and people who present in person.
