Are you a passionate and driven Graduate Social Worker looking for an exciting new challenge?
Grampians Health Ballarat is on the lookout for a dynamic Graduate Social Worker to join our innovative team.
This full-time, 2 year role offers the chance to work across a variety of programs, including acute, outpatients, inpatient rehabilitation, complex care, community rehabilitation, and more.
This position provides an extensive learning and development environment.
The Social Work team provides patients with additional support to improve psychological and social functioning, and we ensure each patients social, emotional and cultural needs are met.
We minimise the negative impacts of illness and hospitalisation on patients.
The focus is on psychosocial assessments, risk assessments, complex discharge planning, patient  and family support.

The Social Work team at Ballarat is made up of 30 plus Social Work clinicians.
You will have support from the Grampians Health Social Work Manager, the Deputy Social work Manager and a team of 6 Senior Clinicians.
