About the Role: Join our team as a Grade 3 Social Worker on an ongoing full time basis, Monday-Friday. The SECU Diversion Team is based within Dandenong Hospital Adult Mental Health Service.
As a member of the SECU Diversion Team, the Grade 3 Social Worker provides comprehensive mental health assessments, develops treatment and relapse prevention plans and works collaboratively with consumers to develop recovery plans. The Grade 3 Social Worker works in a recovery oriented, family inclusive and culturally sensitive manner, and in accordance with:
National Standards for Mental Health 2010The Mental Health & Wellbeing Act 2022Monash Health values and guiding principlesAbout Us: The SECU Diversion Program values and supports each person's unique journey to a life inspired by hope and potential, in the least restrictive manner. We achieve this by providing intensive community care and support to consumers transitioning out of the Secure Extended Care Unit and reintegrating into the community. The Senior Clinician will work with SECU Diversion clinicians, Consultant Psychiatrist, medical officers/registrars and SECU Service Manager to provide direct support, coordination of care, and referral to other services to meet the individual needs of consumers, their carers and family members. The service operates from Dandenong Hospital between the hours of 08:30-17:00 Monday-Friday. Consumers are seen both at the clinic and in their own environments. SECU Diversion provides recovery-oriented treatment for consumers experiencing a range of mental health problems and related psychosocial difficulties. Our clinicians are required to have a good working knowledge of the mental health service structure, including both public health and Mental Health Community Support services, and of the Mental Health & Wellbeing Act 2022.
