About the role
As an Outreach Care Coordinator, you'll play a crucial role in connecting individuals with Alcohol and Other Drug (AOD) and Mental Health (MH) needs to the support they deserve. You will:
Work closely with community agencies, service providers, and individuals who are not currently accessing services. Assess clients' health, wellbeing, and social needs, and coordinate care with a compassionate, holistic approach. Facilitate transitions between services, providing brief interventions and counselling when necessary. Collaborate with multidisciplinary teams and lead programs that enhance access to healthcare for young people and the broader community. This is an opportunity to truly make a difference in the lives of individuals facing complex challenges by ensuring they receive the support they need across various sectors.
About you
We are looking for someone passionate about improving the lives of individuals facing complex needs. You are:
Empathetic, client-focused, and an excellent communicator. Able to engage with a variety of stakeholders, including community groups, health professionals, and clients themselves. Independent, proactive, and able to work collaboratively within a team. Experience in AOD/MH care or outreach coordination is ideal, but the key is your genuine desire to make a positive impact.
