You are encouraged to discuss your needs during the application process.Who are we looking for?We are seeking a passionate First Nations person to join the Community Aged Care Leadership team, supporting AnglicareSA to effectively respond to the cultural needs of First Nations Clients. The First Nations Cultural Practitioner will also provide culturally safe, trauma-responsive interventions to facilitate effective, sustainable change within Community Aged Care.What can you expect to be doing?Assess the quality of care and services being provided in meeting the cultural needs of First Nations customers and inform and manage program reforms.Effectively communicate with First Nations Elders, older people, and their families to promote understanding and provide navigation assistance of the aged care system that maximises their individual choice.Co-design achievable goals, pathways and plans with First Nations Elders, older people, and their families, in a strength-based and self-determining approach.Provide flexible responses and coordinate assistance for individuals and their supporters.Using strengths-based approaches to enable customers to enhance their personal knowledge.Manage a range of caseloads ensuring high standards and quality of work.Lead case conferences to ensure multidisciplinary/multi-agency integrated responses to customer needs and goals, as required.Provide cultural advice.Develop and maintain strong professional relationships with My Aged Care and health care and community services including Aboriginal Community Controlled Organisations and First Nations service providers.What do you need to bring?Must be of Aboriginal and/or Torres Strait Islander descent and identify and be accepted within the community as being Aboriginal and/or Torres Strait Islander.Proven experience engaging and building trusting relationships and co-designing with First Nations Elders, older people, and their families who need support to engage in their community and navigate relevant services.Demonstrated knowledge and understanding of matters concerning the health and wellbeing of First Nations people.Proven leadership experience in service development and providing cultural supervision, guidance, and support to employees and teams that are supporting First Nations customers.Demonstrated track record of being a highly motivated and innovative thinker who is skilled at anticipating issues and confident in escalating, and developing and implementing solutions to complex challenges that result in effective outcomes.QualificationsRelevant tertiary education in health and/or social and community services or demonstrable extensive experience in a related field/sector.Must be of Aboriginal and/or Torres Strait Islander descent and identify and be accepted within their community as being Aboriginal and/or Torres Strait Islander.Who is AnglicareSA?As one of SA's largest social services providers, AnglicareSA makes a positive difference to the lives of more than 50,000 people each year.
