About the role
The In Home Care (IHC) Exercise Physiologist is responsible for clinical care to consumers in both a community setting and Baptcare' s retirement living. Working as part of a multidisciplinary team, the role is responsible for delivering high quality, best practice care that meets the expectation of the consumer and demonstrates a commitment to maintaining an optimal level of dignity, respect, comfort, and quality of life for consumers.
Our ideal candidate will have a passion for clinical rehabilitation, and experience in running group exercise classes at day therapy centres. You will work with various funding bodies, including Home Care Packages, Short Term Restorative Care, CHSP, and privately funded clients.
Key Tasks: The Exercise Physiologist will work within a multidisciplinary team to develop and implement person centred exercise programs for clients with chronic and complex conditions. The work will involve 1:1 as well as group programs. Identify need for additional support services with the customer in care plans. Make appropriate internal and external. Minimisation of infection-related risks through implementing standard transmission-based precautions to prevent and control infection Compliance to all Work, Health, and Safety responsibilities. Report customer risks to management, including inappropriate behaviours or actions not aligned to our values or the Aged Care Quality Standards Ensure that scope of practice is compliant with regulatory guidelines and Baptcare policy and procedures. Provide follow up reviews, interventions, and self-management strategies to empower the customer to achieve agreed goals in line with chronic disease and best practice principles. Provide feedback to customers, referrers and other stakeholders outlining assessment findings, agreed goals and recommendations. About you To be successful in this role, you will need the following:

Demonstrated ability to seek information to support evidence-based practice. Highly organised and adaptable to work in a rapidly changing environment and a consumer's changing circumstances or preferences. Demonstrated ability to make timely decisions and problem solve to ensure quality consumer outcomes.
