Job Description Qualified Social Worker, Knowsley Metropolitan Borough Council, Adults Hospital Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
The responsibilities will include:
Support the discharges from the bed-based intermediate care unit working alongside the Reablement/Rapid Response team Hospital experience would be useful Care Management Review Care Packages Adult Placement Report writing Home Visits Liaise with other professionals 36 hours per week (flexible by negotiation) 2 Month Temporary Contract Start date subject to DBS and reference clearance Benefits Successful candidates will benefit from:
