Applications are sought from suitably qualified Registered Nurses to take the next step in expanding their career and join the Complex Care Team.
This role is perfect for those interested in joining a dynamic team that specialises in Chronic Disease Management.
Key responsibilities include:
Work as a member of a multidisciplinary team, demonstrating well developed clinical skills and the commitment to a holistic chronic disease management service when delivering patient care.
Provides health literacy and self-help coaching to clients with chronic disease in the community.
Works as the primary point of contact to support the intake and triage of new patient referrals.
Demonstrates proficiency in the use and purpose of the Remote Patient Monitoring system and devices.
Maintain Department of Health Key performance Indicators for the Complex Care program under the HIP Guidelines.
Qualifications/experience required:
Registered Nurse with AHPRA.
Post Graduate Qualifications in a specialist field.
Proven experience in acute nursing.
