Social and Health Program Consultant 4 - Complex Case Staffing Specialist The Department of Social and Health Services, Aging and Long-Term Support Administration (ALTSA), Home and Community Services Division, is hiring a motivated and driven individual to organize and lead our work supporting the regions with complex transitions. You'll report to the Senior Strategic Integration Advisor in the Office of Operations, Projects and Integration and join a dedicated group of mission-driven and committed staff.
You'll lead the important work of our agency in tracking and managing staffing processes for complex transitions, partner with other subject matter experts, and support the regions in their work to obtain appropriate quality services to maximize independence, dignity, and quality of life for our clients.
You'll collaborate on the development and application of policy relating to complex cases and will be critical to ensuring the department can follow up on cases and track outcomes. You'll also ensure that our staffing processes are designed and implemented in a manner that meets the needs of the regions and our clients in honoring choice in setting.

You can work primarily by telework anywhere statewide, with occasional in-person meetings in Lacey, Washington.
Some of what you will do: Coordinate with regional and HQ staff on complex cases for clients with exceptional needs who are risking timely transition or their stability in a community. Assist in planning, leading, developing, and managing project development related to staffing complex cases in all regions, focusing on coordinated transitions from inpatient settings to the community. Serve as Subject Matter Expert to regional and Area Agency on Aging staff regarding complex case staffing process and escalation. Participate in task forces, committees, and workgroups to provide program information and consultation related to the coordination of services across the continuum of Long-Term Care. Lead and organize statewide complex staffing, assisting regional and Area Agency on Aging staff in achieving outcomes, navigating changes, and implementing policy or transition practice. In collaboration with other units, develop training materials that enhance staff capacity to navigate complex cases and processes. Who should apply: Those with:
One year of experience as a Social & Health Program Consultant 3. Or two years of experience as a Social & Health Program Consultant 2. Or a bachelor's degree in social work, a health or social science, public administration, or a related field; and 5 years of professional experience in planning, administering, developing, or delivering social, financial, health, or chemical dependency treatment services programs. Or a master's degree in social work, a health or social science, public administration, or a related field; and 4 years of professional experience in planning, administering, developing, or delivering social, financial, health, or chemical dependency treatment services programs. Desired: Programmatic and facilitation skills to guide staff and providers who are gaining skills in navigating benefits accessible across systems. Knowledge of the managed care landscape, the behavioral health system, medical, and long-term care systems in Washington State.
