The In-Reach component will bring the team's community expertise into the hospital care setting. The outreach component will work with individuals with ongoing AOD issues who have had experiences that have impacted them to engage with services.Provide an effective and efficient Social Work/Counselling/Psychologist service that addresses the psychosocial aspects of care for patients receiving substance use treatment including providing support, education and resources as required.
