Accent Home Care offers a range of community-based services, including Home Care Packages (HCP), Commonwealth Home Support Programme (CHSP), and private services.
We focus on preventive care and reducing hospital admissions.
We are seeking to employ an Occupational Therapist who will have the opportunity to work either full-time or part-time as part of our Clinical Home Care Team. A successful therapist will be passionate about caring for the elderly and seek a long and rewarding career with Accent Home Care.
For this position, you will have: Current registration as an Occupational Therapist with the Australian Health Practitioner Regulation Agency (APHRA) to practice with the National Board. Member of the Australian Association of Occupational Therapy (Victoria). Previous experience working with older adults, including their families and carers. Ability to prescribe home modifications and liaise with tradespeople effectively. Ability to prescribe assistive equipment based on functional assessed need. State-wide Equipment Program (SWEP) prescriber endorsement or willingness to obtain. Well-developed communication, planning, organisational and time management skills. Concise report writing skills and ability to provide technical diagrams. Proven ability to establish and maintain effective and cooperative internal and external working relationships. Strong IT skills and the ability to use databases. Ability to create measurable goals based on achievable outcomes for consumers. Ability to work independently and as part of a multidisciplinary team. An understanding of aged-specific diagnoses and relevant interventions. Excellent people skills and a genuine passion for caring for the elderly. Australian Citizenship or Permanent Residency. A valid police clearance (Less than one year old). Be part of a vibrant team. $15,900 salary packaging per FBT year to significantly reduce your taxable income. You will be responsible for: Managing referrals for Occupational Therapy services. Conducting home visits to consumers predominately in the Eastern and Southern metro areas of Melbourne with the possibility of providing a service in the Northern metro of Melbourne. Assess and develop appropriate treatment programs for consumers, engaging with their families/representatives and co-workers to ensure the best possible treatment is given. Assessment and prescription of assistive aids and equipment for resident and consumer rehabilitation needs. Provide goal-based intervention based on assessed needs and care plans. Liaise with medical, nursing, and allied health agencies regarding occupational therapy intervention.
