We are seeking a compassionate and dedicated Clinical Nurse or Mental Health Professional to join our dynamic team.
In this role, you will work closely with older people to provide specialist mental health care, ensuring they remain engaged in the community and reduce unnecessary transfers to acute care settings.
Your expertise will play a vital role in preventing adverse incidents and improving mental health outcomes through collaborative partnerships and high-quality, recovery-focused services.You will coordinate care pathways across the region, deliver direct clinical support, and facilitate access to telepsychiatry services and inpatient admissions as needed.
Your role will involve both direct clinical contact and distance consultation, offering guidance and support to ensure the best possible care for older Australians.Key Responsibilities: Provide specialised nursing and mental health care to older people with mental illnessWork collaboratively to prevent avoidable hospital admissionsAct as a conduit to telepsychiatry servicesLead clinical practice and mentor less experienced staffContribute to the development and refinement of care pathways Essential CriteriaAll applicants must have: Proficiency with Microsoft Office and clinical documentationA commitment to high-quality care and professional growth Clinical Nurse (RN/M2C) Classification: Registered Nurse with a postgraduate qualification in Mental Health and at least 3 years' post-registration experience.
Social Worker or Occupational Therapist (AHP2) Classification: Relevant qualification and professional registration, with experience in mental health clinical practice.
