We are currently seeking a dedicated and compassionate Registered Nurse to join our Transitional Care Unit at the renowned Flinders Medical Centre in Adelaide Metro Southern.
About the role: Reporting to the Nurse Unit Manager (Transitional Care Unit), you will play a pivotal role in ensuring seamless transitions for patients moving between acute care and home or residential care settings. Duties for the role will include:
Providing high-quality nursing care to patients with complex healthcare needs, ensuring their comfort and well-being.Collaborating with an interdisciplinary team of healthcare professionals to develop and implement comprehensive care plans.Educating patients and their families on post-discharge care, medication management, and community resources.Maintaining accurate and comprehensive patient records, adhering to best practices and quality standards.About you: Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.Exceptional clinical skills and a genuine passion for delivering patient-centred care.Effective communication, critical thinking, and a commitment to ongoing professional development.Please see the attached Role Description for additional information.
