We are seeking a Community Health Nurse – Rehab to join our team! About the Role This position works within the Primary Health Team with a focus on providing health education and early intervention, both on an individual and group level.
This will include (but not be restricted to) providing direct care to clients with chronic conditions and assisting them with self-management strategies.
This position would also identify and prioritise local community health needs by planning, implementing, and evaluating programs (both current and future).
The position will provide a range of primary care initiatives and direct client services, consistent with the strategic direction of PCC and in line with best practice standards.
This position reports to the Team Leader of Nursing Services within the Health Services team and works within the Primary Health Team.
It is also expected that this position works within the larger Primary Care Connect team, and networks with external organisations.
Key Selection Criteria The following criteria must be met and addressed in applications for consideration for this position:
Registered Nurse with a Current AHPRA RegistrationMinimum 2 years of experience as a Community Health NurseA thorough understanding of the principles underpinning primary health care and health promotionExcellent knowledge and understanding of working with clients to achieve social, emotional and physical wellbeing with an emphasis on the prevention, management and self-management of diseaseExperience working with clients from linguistic and culturally diverse backgrounds, with clients who may have a disability and/or clients with limited literacyExperience in the delivery of a range of chronic disease education programsExcellent clinical assessment skillsAn understanding of the social determinants of health and health promoting principlesDeveloped written and verbal communication skillsExperience using technology to complete daily work tasksProfessional and personal alignment with Primary Care Connect core valuesBenefits and Perks Primary Care Connect provides a supportive workplace with staff who are committed and passionate about the community that we work in.
