In-home clinical assessments, reviews and care needs for clients With support from the Community RN, provide clinical advice and support to clients and liaise with services for referrals where more specialized support/advice may be required Work closely with local allied health professionals to facilitate responsive care in our clients homes Permanent part time opportunity, 3 days a week during business hours Balance of in office and home visits depending on client and organisation needs About you: This is the place for you if you have: Diploma of Nursing or equivalent qualification with current AHPRA Div 2 Nursing registration (required) Minimum 3 years experience as an Enrolled Nurse in Community, Commonwealth Home Support or District Nursing (required) Team player, flexible in your approach to work with a positive outlook Experienced in actively visiting clients in their homes Experienced with evaluating and implementing care support plans Familiar with the My Aged Care portal Proficiency in decision making, critical analysis and on the spot problem solving Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions Availability to work 3 days a week on a permanent roster Own your own registered and insured vehicle (travel reimbursement between clients is provided) Thrive in a team and also appreciate the one to one interactions Are committed to being part of a workplace culture based on the B E S T way of life (Believe, Excel, Support and Trust) Essentials for working in Aged Care: A current Working With Children Check card Current and ongoing required flu and COVID-19 Vaccinations Australian working rights Ability to pass a national police clearance and NDIS Clearance Make a positive difference in your community – apply now!
