CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond.
The service provides beds across three campuses (Sale, Maffra, and Heyfield).
Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical services and sub-acute services including rehabilitation.
CGH is also a leading teaching hospital, affiliated with Monash University's School of Rural Health and provides placements and clinical experience for students.
About Department Within the Community and Allied Health Services directorate, the Home Support team provides care and support to our community in a range of settings.
The main setting is the client's home.
Other settings include CGH centres and in the community.
The client group includes older people and people with a range of disabilities.
About the Role As a qualified Community Care Worker, you would be supporting older people and people living with disabilities to remain living independently at home by providing excellent home and community-based care and services.
The CGH approach to providing these services is to place the client at the centre of their care and support them to achieve their personal goals.

We are looking for qualified Community Care Workers to support clients in the following localities and surrounds: Sale, Maffra, Heyfield, Rosedale, Loch Sport.
Duties Complement the client/carer skills and abilities by assisting them with domestic assistance which includes cleaning, vacuuming, sweeping and mopping floors; ironing and washing clothes; moving light furniture; bed making; and assistance with meal preparation.Provide personal care including assistance with showering, bathing, dressing/undressing, toileting, grooming; assisting with medications, eating, and drinking.Provide respite care in the client's home and in community settings.Provide clear and concise documentation related to service delivery, including progress notes.Qualifications, Skills and Experience Minimum Certificate III Individual Support – Aged Care or Disability (or equivalent) or working towards obtaining the qualification.Current Victorian Drivers Licence.Application Please attach your resume and cover letter to your application.All appointments are made subject to the candidate providing a current satisfactory National Police Certificate, NDIS Workers Screening Check Clearance, and obtaining a Working with Children Check.
