We're looking for a dedicated Commonwealth Home Support Program Coordinator to join our team and make a real difference in the Eyre & Far North region.
About the Role:
As a Commonwealth Home Support Program Coordinator, you will play a crucial role in delivering a comprehensive Community Care Program.
Your responsibilities will include:
Identification of Needs:
Assessing the needs of clients to tailor services that support their unique requirements.
Assessment & Implementation:
Conducting thorough assessments and implementing individualised care plans.
Coordination & Maintenance:
Overseeing the coordination and maintenance of home and community-based programs.
Evaluation & Review:
Regularly evaluating and reviewing care programs to ensure they meet the highest standards and effectively support our clients.
Key Responsibilities:
Collaborate with a multi-disciplinary team to develop and deliver community-based aging and disability programs.
Work within the parameters and criteria set by the Eyre & Far North Local Health Network (EFNLHN).
Ensure that clients receive the right support to maintain their independence and quality of life while living at home.

This is a part time vacancy working 60 hours per fortnight and salary will be adjusted according to actual hours worked.
