We are seeking a passionate CHSP Coordinator (Commonwealth Home Support Coordinator) to support clients receiving services under the Commonwealth Home Support Program while they live independently in their own homes.Key ResponsibilitiesConduct client assessments and develop tailored service plans that prioritize wellness and client goals.Manage and resolve client and employee inquiries efficiently.Ensure compliance with funding body legislation and maintain service quality.Foster strong relationships with external partners and contribute to the strategic direction of Goodwin Home Care.Deliver exceptional program services while maintaining accurate documentation.Essential QualificationsCertificate IV or higher in Service Coordination (or similar) and/or extensive experience in CHSP service coordination.First Aid Certificate and a current Driver's License.Strong knowledge of the Home Care sector, including aged care and CHSP protocols.Ideal CandidateDemonstrates strong organizational and problem-solving skills.Exhibits excellent interpersonal and communication abilities.Adapts well to changing environments and is committed to continuous improvement.Respects cultural diversity and maintains high ethical standards.If you are enthusiastic, detail-oriented, and passionate about enhancing the lives of aged individuals, we want to hear from you!
