We are seeking a dedicated and passionate Homelessness Commissioning Coordinator to spearhead initiatives aimed at breaking down barriers to primary health care.

Your focus will be on commissioning innovative services that address the unique health and social challenges faced by individuals experiencing homelessness.
By fostering strong collaborations with health, social, and community service providers, you will work towards creating a network of support that prioritizes person-centered care.
Your efforts will directly contribute to improving health outcomes and promoting equity within our communities, ensuring that everyone has the opportunity to thrive, regardless of their housing situation.

The successful candidate will have: Tertiary qualifications in health, social work, public health, or a related field.Demonstrated experience in commissioning, program management, or service delivery within the health or homelessness sectors.Strong understanding of the barriers faced by people experiencing homelessness in accessing primary care, and the role of integrated health services in reducing inequities.Proven experience working with vulnerable populations, including people experiencing homelessness, Aboriginal and Torres Strait Islander communities, and those with complex health needs.Excellent interpersonal and communication skills, with experience in stakeholder engagement and service co-design.Project management experience, with a focus on service integration, data-driven decision making, and outcome evaluation.How to apply: Address the selection criteria (both essential and desirable) in your cover letter.

Additional Information*PHN's have been funded by the Department of Health and Aged Care since 2015.
This is a maximum term contract until 1 November 2025 and any contract extensions are dependent on ongoing funding.
**Salary: This role is classified as I – M within our Enterprise Agreement and the rate will be dependent on skills and experience/qualifications.
