Exciting Opportunity: Clinical Practice Consultant - Palliative Care RN3- Adelaide Join our compassionate team in Keswick, South Australia, as a full-time Clinical Practice Consultant (CPC) in Palliative Care.
This permanent role includes travel to various sites as needed.
Role Overview: Clinical Practice Consultant will play a crucial role in providing clinical leadership, guiding nurses in the development, execution, and assessment of evidence-based, patient-centered care models.
You will oversee training, manage clinical practices, and conduct individual competency assessments for staff assisting clients with complex health challenges in the community.
Your contributions will be vital in enhancing clinical care, service efficiency, and client outcomes.
About the role: Deliver high-quality clinical care to clients and their families, utilizing evidence-based methods.
Share clinical expertise through policy formulation and the execution of evidence-based practices.
Collaborate with stakeholders to achieve optimal results for clients.
Develop, implement, and coordinate training initiatives for staff.
Monitor and evaluate client health support plans to ensure positive outcomes.
Maintain precise clinical records.
Offer specialized clinical insights and interventions for clients with intricate needs while adhering to professional standards and a client-focused approach.
Assess, plan, implement, and coordinate care independently.
About You: You are committed to excellence and passionate about positively impacting others' lives.
To excel in this position, you should have: Registration in AHPRA Post Graduate in Palliative Care Nursing + 2 years Post Graduate Experience Background in a leadership or managerial capacity.
Specialized skills and knowledge relevant to community nursing and the demands of this role.
Advanced expertise in planning and executing nursing care relevant to your responsibilities.
Proven experience in providing clinical leadership in a specific area.
In-depth understanding of Primary Health Care Principles, Chronic Disease Management, and Self-Management strategies.
Proficiency in necessary software and systems.
