DPV Health is expanding the Pathway to Good Health service for vulnerable children and young people in Out of home Care, in partnership with the Department of Health and DFFH Child Protection. As part of the statewide rollout of the program DPV Health will provide care across the Northern Metropolitan Melbourne with clinic locations in Broadmeadows and Mill Park, as well as community in-reach and outreach.
The Nurse Coordinator is responsible for referral management, triage, health history collation and identification of unmet health needs for children and young people in care. The position supports health system navigation and coordinates care in partnership and collaboration with DFFH Child Protection and members of a child's care team to support improved health outcomes. The incumbent will use clinical expertise and a trauma informed approach to contribute to comprehensive health assessments and production of holistic Health Management Plans, as part of a multidisciplinary team.
Working as part of the Pathway to Good Health multidisciplinary team, and reporting to the Clinical Lead, you will:
Act as an interface between the Child Protection system and health services, ensuring cross sector collaboration, providing health service navigation and capacity building.Conduct secondary consultation with DFFH Child Protection North Division and other partners to provide clinical triage, identify unmet health needs, provide individualised clinical advice, and recommendation formulation.Gather health and social information in line with information sharing legislation to collate a health history, utilise relevant records to interpret, review and communicate clinical information.Identify and navigate barriers to service engagement by offering a flexible model of care.Collaborate with the multidisciplinary team to conduct comprehensive health assessment and contribute to the production of holistic Health Management Plans, aligned with the National Clinical Assessment Framework for Children and Young People in Out of Home Care.Conduct referral management and care coordination to ensure timely facilitation of multidisciplinary assessment and initiation of referral to other health services as appropriate.Participation in multidisciplinary clinical case conferences and care team meetings with external partners involved in a client's care team to proactively ensure collaborative care.Provision of health promotion, health education, psychoeducation, advocacy, social prescribing and support for clients and their caregivers.Working in partnership with clients and caregivers to build health literacy and ensure clients are engaged in shared decision making about their health goals and care planning.Desired Skills and ExperienceBring your great, everyday! To be successful in this role you will bring:
Current APHRA Registration as Division 1 Registered NursePaediatric nursing experience of at least three years post-registration and post graduate qualification in speciality area highly desirable.Experience working with vulnerable children and adolescents and culturally and linguistically diverse clients desirable.Well-developed paediatric and adolescent clinical assessment skills and a sound understanding of child development, attachment theory and the social determinants of health.Ability to work within a highly sensitive environment and an understanding of the effects of trauma and child maltreatment and the principles of trauma informed care.A positive regard for and a commitment to supporting children and young people facing adversity.Demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes in collaboration with children and young people, caregivers, biological parents and a child's wider care team.Ability to work autonomously within scope or practice and meet agreed timelines as well the ability to work within an inter-professional, multi-disciplinary framework.The fine print Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), NDIS clearance and hold a valid WWCC.Full driver license and travel between sites required.Closing date for applications will be 20 th December 2024Our Talent Experience We kindly request you include a covering letter, addressed to Kate Brownlee detailing your motivation for joining DPV Health and highlighting the qualities you bring to this position.
