About the role: The successful candidate will deliver high-quality, specialised palliative care consultations across inpatient, outpatient, and residential care settings, with a focus on improving outcomes for rural and regional patients. This role involves travel and telehealth support to remote areas, acting as a leader in clinical practice and service delivery. The candidate will demonstrate strong clinical assessment, case management, and communication skills, alongside a passion for educating and guiding the rural and regional workforce. They will contribute to best practices in palliative care and ensure safe, effective autonomous practice.
In addition to clinical duties, the candidate will play a key role in advancing service provision and implementing guidelines, policies, and models of care. They will receive operational support from their line manager and clinical guidance from the Palliative Care stream network. Ongoing professional development will be encouraged through clinical supervision and education opportunities, including presenting at workshops and conferences. The candidate will embody the values of integrity and professionalism while working toward fulfilling the five domains of the CNC role.
Where you'll be working: The highly dedicated team within the palliative care service will look to you for guidance, clinical expertise and support.
