About the role

As the Clinical Lead - Home Care with Home Instead Melbourne Outer North East, you will play a vital role in delivering exceptional in-home care and support to older adults in the Melbourne Outer North East area. This part-time or full-time position (negotiable) offers the opportunity to apply your nursing expertise and leadership skills to ensure our clients receive the highest standard of personalised, compassionate care. You will work closely with our care managers to coordinate care plans, provide clinical oversight, and foster a culture of excellence.
What you'll be doing Assess and develop individualised care plans for clients to address their unique health and wellbeing needsProvide on-site clinical supervision, support and mentoring to our Care Managers and CAREGiversMonitor client progress and adjust care plans as required to ensure optimal outcomesLiaise with clients, their families, and healthcare providers to coordinate holistic careEnsure compliance with all relevant nursing, clinical, and safety standardsSupport the continuous improvement of our home care servicesWhat we're looking for Registered Nurse (Division 1) with current AHPRA registration5+ years of nursing experience, preferably in aged or community careStrong clinical assessment, care planning, and case management skillsExcellent communication and interpersonal abilities to engage with clients, families, and the care teamDemonstrated leadership experience and the ability to mentor and support a teamCommitment to providing high-quality, person-centred careWhat we offer
