We will be offering fixed term contract positions, in line with Aged Care government funding.
We have a Part-Time position available within our Clinical Aged Care Assessment team (ACA) in Bega and the surrounding region.
There may be opportunities to increase the number of working days in a nursing capacity, providing services such as wound care to our vulnerable clients.
Travel is an expectation in this role, which includes driving in and around Bega and surrounds or to other areas as required, conducting face-to-face assessments in the client's home.
You will be reporting to the Team Leader and must also be able to work autonomously.
The role responsibilities include: Completing face-to-face assessments to determine older Australians' eligibility for support within the Clinical and Non-Clinical Aged Care space.Reviewing previously assessed clients.Undertaking client-centred support planning using a wellness and reablement approach.Liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require.Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.KPI based.Preferred candidates will have: Registered/Enrolled Nurse experience.
Will also consider applicants with an allied health background, or other roles or experience with similar transferrable skills.Current AHPRA registration.Completed My Aged Care, Home Support Assessor Training (SOA2) or willingness to obtain.Ability to work independently, and as part of a team.Experience in meeting KPI's.High level of computer literacy skills.Ability to follow instructions and guidelines relevant to the position as well as demonstrated problem-solving capacity.Valid NDIS Worker Check and Working with Children Check (NSW or equivalent).Current First Aid certificate.Interchange is committed to being an equal-opportunity employer and promotes a diverse and culturally safe environment.
