As the Care Coordinator, you will be responsible for providing advanced clinical support, leadership, and coordination of clinical care for residents/clients with complex needs.
You will contribute as an integral member of the Seniors Living group and support activities aligned with the strategic direction of and consistent with the mission and values of Churches of Christ.
In this role you will: Oversee and co-ordinate the completion of baseline and required focus assessments of individual residents/clients' needs and record findings within a Positive Wellbeing Model;Ensure that care plans are developed and reviewed based on the assessment findings of individual residents/clients' needs, in accordance with the organisation's timelines;Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;Monitor that resident/client documentation is accurately reflective of care provisions and is in accordance with organisational requirements;Co-ordinate the care provision of external service providers;Assist in the review and maintenance of clinical programs and initiatives;Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.The role offers the following availability: workdays are Monday to Friday, with every second weekend required.
Flexible start time is either 7am or 8am.

As the Care Coordinator, ideally your experience and qualifications include: Current AHPRA registration as a Registered Nurse;Three years post-graduate experience in aged care or acute clinical setting;Experience in managing and leading teams in the aged care sector;Superior communication skills both written and verbal.Pre-requisites: Current driver's licence;Possess or have the ability to obtain a National Police and NDIS Screening Check;National Personal Insolvency Check or the ability to acquire.Importantly, you will be someone who closely aligns with our organisational values of: Unconditional Love, Continual Innovation, Mutual Trust, Wise Stewardship and Safety.
