We have an exciting opportunity for a Registered or Enrolled Nurse who speaks Mandarin and Cantonese to join us as a Clinical Care Nurse. If you are looking to make a real difference for ageing Australians in the Chinese speaking community who wish to stay in their own home, we are offering a permanent, full time, office-based role. Working 37.5 hours a week in a telehealth model, 3 days in the office in Clayton and 2 days from home.
As our Clinical Care Nurse, you will: Work with the Senior Manager Advice & Planning to partner with Care Managers to provide clinical assessments. Liaise with team members on concerns and complex care needs and engage primary or other health professions where appropriate. Consult with clients, their carers/families and other supports to ensure that the care is ongoing and that all client needs are identified and met. Monitor the changing needs of clients and deterioration of their health, work with the Care Managers to identify the clients' care path and escalate any deterioration of health and risks associated. Utilise brokerage nursing services for continence management, wound care and medication and injections, ensuring ongoing assessments, reassessment requests and follow ups are completed in a timely manner. Occasional site visits to clients may be required. We are looking for a Nurse with: Fluency in Mandarin and Cantonese. A current Nursing Qualification, including a current unrestricted AHPRA Registration. Extensive clinical experience – ideally in aged care. Strong interpersonal and verbal/written communications skills and the ability to develop relationships with clients, their carers/families and employees. Experience with CRM systems and Incident Management systems, with good computer literacy skills and capabilities. Sound knowledge of the Age Care Quality Standards. The right to work in Australia.
