Clinical Care Coordinator (RN) Aged CareSignature Care presents an exciting opportunity to shape the service delivery of aged care into the future during a period of significant change.
We are currently seeking an experienced and qualified Clinical Care Coordinator to work Permanent Part Time, 8 shifts per fortnight, to work closely with our Clinical Manager, Deputy Residential Manager and Residential Manager to develop our stunning 180-bed aged care residence based in Maryborough.
The Clinical Care Coordinator (CCC) will provide day to day clinical leadership to the team, foster a culture of clinical excellence and oversee the clinical care of our residents to ensure that a high standard of care is maintained.
Skills and requirements for this role: Ideally at least 2+ years experience in a Clinical Care Coordinator or similar role in the aged care or acute sector;RN qualification with current AHPRA registration;Excellent and up to date clinical knowledge and assessment skills;Experience in staff supervision and development;Strong communication and negotiation skills;A working knowledge of funding and accreditation processes;Knowledge of Care Planning.Come and join our friendly and supportive Signature Care team that offers:
