Our client is seeking a dedicated Full-Time Clinical Care Coordinator to join their compassionate team at Wangaratta, nestled in the beautiful Yarra Valley & High Country region of Victoria. Position: Clinical Care Coordinator Location: Wangaratta, Yarra Valley & High Country, VIC Industry: Nursing - Aged Care Employment Type: Full-Time Salary: $120+Super Additional Benefits: Relocation Assistance and Initial Housing About the Role: As a Clinical Care Coordinator, you will play a pivotal role in providing clinical leadership and ensuring the delivery of top-notch care services. Working closely with the Facility Manager and other key stakeholders, you'll oversee and guide the implementation of resident-focused clinical strategies to achieve positive outcomes. Key Responsibilities: Utilize your robust clinical skills to maintain high standards of care delivery. Provide leadership, mentorship, and supervision to our clinical team. Conduct comprehensive assessments and develop tailored care plans for residents. Ensure compliance with AN-ACC funding model and Aged Care Quality Standards. Requirements: Current registration with the Australian Health Practitioners Agency (AHPRA). Proven experience as a Registered Nurse in Aged Care with strong clinical acumen. Previous experience in a residential care setting is essential. Genuine passion for enriching the lives of elderly individuals. Familiarity with AN-ACC funding model and Aged Care Quality Standards is desirable.
