What You Will Be Doing The Health Independence Program (HIP) supports people in the community, in ambulatory settings and in people's homes, to improve and optimise a person's independence and participation in in the community. The Grade 2 HIP physiotherapist will work within the Community Falls Prevention Service and the Community Care multidisciplinary outreach program. The Grade 2 Physiotherapist will provide comprehensive assessment and management of clients at risk of falls and with complex health care needs. The physiotherapist will also be responsible for coordinating intensive, short-term interventions to clients with chronic health conditions and/or complex health needs. This role will facilitate collaborative, integrated care planning across acute and community-based services. What You Need We anticipate you will have: Essential Criteria • Current registration with AHPRA as a Physiotherapist / Occupational Therapist. • Current Australian Driver's license. • Demonstrated experience in providing comprehensive Physiotherapy / Occupational Therapy assessment and rehabilitation of older clients. • High level of verbal and written communication skills. Desirable Criteria • Experience in assessment and management of falls risk in older adults living in the community. Experience in delivering home based interventions. • Interest or experience in quality improvement activity.
