Role Responsibilities: Collaborate with families and medical professionals to assess clients' needs.Develop and coordinate care plans, ensuring smooth continuity through efficient scheduling.Conduct home visits to clients for ongoing support.Review and update client records regularly to ensure compliance with care standards.Offer guidance to the care team and build strong, collaborative relationships with healthcare professionals.Lead efforts in quality improvement and service enhancement initiatives.Required Skills & Experience: Current AHPRA Registration or an International Nurse currently awaiting AHPRA Registration.Minimum 3 years experience as Registered NurseStrong case management skills, especially in handling complex needsCapacity to respond crisis situations, using well developed conflict resolution, negotiation, and networking skillsExcellent computer skills with a minimum of 60 WPM, organisational, communication, and clinical knowledgeCurrent certifications in CPR, Manual Handling, and vaccinations (COVID-19 and Influenza)Current NDIS screening Check, Working with Children check and Police check
